9CR Forms

In order to use this form, you must *left* click on the form number (do NOT *right* click as you normally do to download a form!) and be patient, as the form itself must be copied into the browser and Reader has to start. You may experience a white screen for a period of time- that's normal. When you have finished filling out the form, you should print a copy of the completed form by using the printer icon in the Reader window. Once you click on the "Submit" button, the data will be sent to the Coastie operator and an acknowledgement screen will appear confirming what was sent.

FLOTILLA COMMANDER ACADEMY REGISTRATION FORM

When you have finished filling out the form, you should print a copy of the completed form by using the printer icon in the Reader window. Once you click on the "Submit" button, the data will be sent to the VCO and an acknowledgement screen will appear confirming what was sent.

In order to use this form, you must *left* click on the form number (do NOT *right* click as you normally do to download a form!) and be patient, as the form itself must be copied into the browser and Reader has to start. It must be filled out while on-line- if filled out while NOT on-line, the data cannot be submitted electronically. Division Captains need to enter their staff and their Division's Flotilla information (only FCs and VFCs). All contact information will come from AUXDATA (with the exception of the officers' names, which are to be entered in UPPER and lower case as indicated on the form). No address-phone-email information is needed (pretty cool, eh?).

Division information will automatically go to the DSO-IS for AUXDATA entry. Complete Flotilla information will have to be submitted later.

Please double-check member numbers, as that is the field that is used to search AUXDATA!

Acrobat Reader 5 or above is needed for these forms to function correctly.
